Topic: System Safety – The other side of hazard management

When we think of hazard management, we often think of the control of hazards directly affecting the activities of workers in the workplace; how to provide an environment that allows workers to do their job safely. System safety involves looking at infrastructure or activities as part of a larger system, identifying hazards and analyzing for probability and severity of occurrence, and mitigating the associated risk to an acceptable level. Prevention through design is practiced where possible, but system safety can (and should) be applied to existing systems. In this presentation John will talk about the practice of system safety as applied to rail transit systems, provide some real-world examples of system safety successes and failures, and hopefully give the audience a better understanding of risk acceptance in system development that can be applied to similar aspects of our responsibilities as safety professionals.

John has enjoyed over 33 in the rail industry, working his way up from a track laborer and then switchman/brakeman to locomotive engineer, train dispatcher, operations manager, superintendent, and then rail safety officer for Caltrain. For the last 11 years John has been consulting for several large engineering firms, applying his rail operations and safety experience to projects such as the development of the California High Speed Rail system, installation of a new train control system at Caltrain, rehabilitation of aging BART infrastructure, system safety support for TriMet light rail system in Portland, and currently the safety and security certification of the BART extension into downtown San Jose. John has been a member of ASSP for over 12 years and has lived in Berkeley with his wife and son for 24 years. In his free time John enjoys hiking, mountain biking, photography, and model trains.
